2-1-1 San Diego
2‑1‑1 is a free telephone number providing access to local community services. 2‑1‑1 is available in multiple languages, allowing those in need to access information and obtain referrals to physical and mental health resources; housing, utility, food, and employment assistance; and suicide and crisis interventions.
ADL/IADL Checklist
The ADL/IADL Checklist is part of the Dementia Care Aware cognitive health assessment and can be printed or downloaded.
ALZ Companions Respite 
ALZ Companion visits help both care partners and people living with dementia by connecting them with a friend that they can visit weekly. They carefully match the person with a volunteer based on factors like location, personality, and/or interests.
ALZ Direct Connect® Alzheimer's Los Angeles
This service allows healthcare providers to directly link patients and families to Alzheimer’s Los Angeles where they can receive care coordination and psychosocial support, referrals to supportive services (often at no cost), help understanding the disease & navigating its progression, and a 360 approach to care through feedback to the referring provider.
