The Age-Friendly Health Systems Initiative is a movement that aims to improve age-friendly care using the 4Ms (What Matters, Medication, Mentation and Mobility) Framework to provide patients, families, health care providers and systems with the tools they need. Resources are available in English and Spanish.
Age Well Senior Services
Age Well Senior Services provides resources to older adults in Orange County that support their independence and promote health & wellbeing. They provide nutrition (Meals on Wheels), transportation (Congregate), Adult Day Care Classes and Care Management services.
Aging & Independence Services (San Diego County Area Agency on Aging)
San Diego's federally-designated Area Agency on Aging provides many state and local programs. They offer assistance, information, and referrals to older adults, those with disabilities, and their family members.
ALZ Companions Respite 
Alzheimer's San Diego's ALZ Companion program helps both care partners and people living with dementia by connecting them with a friend that they can visit with weekly. People living with dementia and care partners are carefully matched with a volunteer based on factors like location, personality, and/or interests.
