Bold Dementia Detection aims to build understanding of dementia as a manageable chronic condition through effective public health, clinical, and community initiatives.
CAP-CT Horizontal
Caregivers As Partners in Care Teams (CAP-CT) is a national training program that provides health care teams with the skills and confidence to include family caregivers in a patient's care journey. Through training and practical resources, health care teams can learn to identify, engage, and support caregivers, leading to better care for patients and their caregivers.
CHAMPPS
The CHAMPPS project is an extension of the Dementia Care Aware program that was created to investigate the impact of the Cognitive Health Assessment (CHA) in primary care for underserved populations. Specifically, CHAMPPS will look at how new screening programs in primary care using the CHA as the screening approach has led to changes in the number of patients with dementia diagnoses, the content and quality of their evaluations and care plans, and if there are disparities in dementia diagnoses and care.

Within Dementia Care Aware, the Medical-Legal Partnership Network (MLP Network) has partnered with legal aid organizations and health care systems across California. The MLP Network has developed trainings for primary care teams on common legal issues that affect people with cognitive impairment.

Cognitive Health Assessment Metrics Post Positive Screen (CHAMPPS)

The CHAMPPS project is an extension of the Dementia Care Aware program that was created to investigate the impact of the Cognitive Health Assessment (CHA) in primary care for underserved populations. Specifically, CHAMPPS will look at how new screening programs in primary care using the CHA as the screening approach has led to changes in the number of patients with dementia diagnoses, the content and quality of their evaluations and care plans, and if there are disparities in dementia diagnoses and care. 

CHAMPPS is a 4-year research project ending in June 2028 and funded by the California Department of Public Health A144252.  We are a consortium of grantees at the University of California San Francisco, The Lundquist Institute, and Rancho Research Institute.
