The Dementia Care Aware Warmline is designed to provide decision-making consultation for clinicians and primary care teams in California around dementia screening, assessment, diagnosis, management, and care planning.

We promote the Dementia Care Aware cognitive health assessment as a screening approach, but consultants can answer questions that arise during any part of dementia care including related medical legal considerations and how to implement systems change. We also provide support around operational challenges using the cognitive health assessment or implementing dementia screening and care, e.g., billing questions, documentation tips, etc.
