This Dementia Care Aware webinar, Dementia Care Aware and Age-Friendly Recognition: An FQHC Case Study, is the thirty-sixth in the series. In this session, Dementia Care Aware, LifeLong Medical Care, and the Institute for Healthcare Improvement (IHI) explore how age-friendly care principles can be translated into ambulatory clinical settings.
